Patient-Centered Medical Home

PHILADELPHIA (CBS) - It is called the patient centered medical home and it is one if the hottest concepts in medicine.
Essentially the patient centered medical home is a way to describe doctor's practices that provide first contact, continuous, comprehensive, whole person care for patients across the practice.
The keys are: care of the whole person - where the doctor provides the first contact and continuous comprehensive care - and it is best when developed with a supporting organization and a team.
The goal is to get patients involved in controlling their care but with close follow up.
