Lourdes Health System Partners With Walgreens To Get Prescriptions To Patients Before They Leave The Hospital

CAMDEN, N.J. (CBS) - A pharmacy chain and local health care provider are trying to prevent hospital readmissions caused by medication issues.
Our Lady of Lourdes Health System in Camden is partnering with Walgreens Pharmacy to make sure patients being dishcarged have the prescriptions they need in hand and take them properly.
Hospital COO Mark Nessel says patients can get the medications delivered to their rooms before discharge, and the pharmacist will make follow up calls.
"If there are any questions of medication issues when they're home than the pharmacist when they make those calls they can discover them and head them off before there's an event or the patient has to come back into the hospital."
He says all too often people leave the hospital and head home, without stopping by the pharmacy to get the medications they need to keep them healthy.
