Camden Diabetes Program May Help Nation Improve Care

CAMDEN, NJ (CBS) -- A Camden diabetes program may be a national model for improving care.
Nadia Ali, the program manager for the Camden Citywide Diabetes Collaborative, says patients with diabetes are twice as likely to go to the ER as patients without diabetes.
"And then once they're in the ER, patients with diabetes are twice as likely to be admitted so that just leads to a lot of escalating costs and low quality, fragmented care. And so the goal of this program that we've launched -- it's really for patients with diabetes to get better, more individualized and more coordinated care from their primary care providers," Ali explains.
Ali adds that they do that through home visits.
"For a team to go to their home and really understand them and see where they're coming from and target the care based on what their lifestyle is like, it's been very rewarding," she says.
