The Pentagon said that numerous failures led to the October 2017 attack in Niger that left four U.S. soldiers dead. It was the largest loss of American troops in Africa since the 1993 attack in Somalia. The military released a video recreating the Niger attack and published the findings of a monthslong investigation.
