Bridget Erwat is accused of leaving an 8-month-old girl and a 4-year-old girl alone in a locked car in the Costco parking lot in Melville on Monday afternoon.
A passerby, who noticed the two girls alone in the backseat, unlocked a door through an open window and brought the children into the store for safety, police said.
Investigators believe the girls, who were not injured, were in the unattended car for about 10 minutes.
Erwat, who runs Bridget's Children's Place at 26 Ira Court in Amityville, has been charged with two counts of endangering the welfare of a child.
The 49-year-old posted bail and will be arraigned at a later date, police said.
The Suffolk County Department of Social Services Child Protective Services Bureau has been notified.
