In the filing, Longoria, who had taken Parker's name, is requesting that her name be restored to Eva Jacqueline Longoria. The filing also states that the couple had a prenuptial agreement.
A source told People magazine that Longoria was "totally and completely blindsided" by reports that surfaced this week claiming Parker was unfaithful.
The couple got engaged in 2006 and married on July 7, 2007, at a 17th-century castle in France. They have no children together.
