At a news conference Wednesday, MBTA interim general manager Jonathan Davis said the driver was coming off an eight-hour night shift at another job when he went to work driving a Green Line trolley at 11 a.m. last Thursday.
The crash happened less than an hour later.
Davis said the driver, who he would not identify, had failed to report his second job, and showed up for work unfit for duty because he was fatigued.
There is no rule prohibiting operators from holding second jobs, but Davis said the driver had an "alarming disregard for customer and employee safety."
He did not say whether the driver may have fallen asleep at the controls before slamming into a parked trolley inside the station.
About three dozen people were injured in the crash, which caused about $500,000 worth of damage to the trolleys.
