MIAMI (CBSMiami) --Blood stains and shattered glass littered the floor of the home of former Miami-Dade Mayor Carlos Alvarez in newly released pictures.

It's something police say was caused by his ex-girlfriend Evelyn Fernandez -  a lieutenant with Miami-Dade Police.

The 44-year old was arrested over the weekend in connection to the case prompting her to be relieved of duty with pay pending the outcome of the investigation.

Pictures released Monday showed a number of glass shelves shattered. What appears to be some sort of towel filled with blood stains is pictured laying on the floor among the shattered glass. Blood stains could also be seen on an area rug.

CBS4's Peter D'Oench said according to a new police report, the blood came from Fernandez's shin. She was later transported to a hospital for treatment before being taken to jail.

According to the report, a witness saw Fernandez hitting a rear window of an apartment with a floor tile and said Fernandez walked towards the building's pool and threw the tile in the pool. The witness did not see Fernandez after that.

Officers said they responded to Alvarez's home just before 12:30 a.m. on Sunday to investigate a possible domestic violence incident at the apartment located at 55 Merrick Way.

Once inside, police found Fernandez, had damaged a number of items. Her arrest report says there was more than $5,000 worth of damage at the apartment, according to Alvarez.

Fernandez told police she did not have a set of keys to the home. Officers noticed there was no forced entry into the apartment. It's something that puzzles residents who do not know how Fernandez could have gained access to the apartment.

Stephanie Fichera said, "It's pretty difficult. You have key FOBs that you need to get in any door. There is security here 24-7 ad there is a guy at the front desk and also a security guard."

Officers arrested Fernandez and transported her to the Turner Guilford Knight Correctional Center (TGK) on charges of burglary and criminal mischief. She was released Sunday and is under orders to stay away from Alvarez. Fernandez did not have to post bond since she was released under a court program.

Fernandez told CBS4 the charges against her are "false."

The incident is just the latest in a string of messy domestic-related issues the former couple has been involved in during their 14-year relationship.

In April, Alvarez, 63, was charged with battery following an alleged altercation with Fernandez. She said verbal and physical abuse by Alvarez escalated in 2013 after he began a bodybuilding career.

Alvarez pleaded not guilty in April's case and is awaiting a court hearing.

He told CBS4 that he could not comment right now about this latest case.

Alvarez was named director of Miami-Dade Police in 1997 and elected county mayor nine years later. However, following a raise in property taxes and the controversial plan to use public money to build Marlins Park in Little Havana, amid other criticism, he was recalled as mayor.
