Baltimore Agency Among 5 Cited In N.J. Factory Worker's Crushing Death
AVENEL, N.J. (AP) -- Federal labor officials have cited five companies connected to operations at a New Jersey-based Amazon packaging facility for serious violations following the death of a worker.
The U.S. Department of Labor's Occupational Safety and Health Administration said Thursday that the contractor responsible for operating the facility in Avenel and four temporary staffing agencies were cited in the death of temporary worker Ronald Smith.
The four staffing agencies cited are in Baltimore, New York, California and Ohio.
Smith died from injuries sustained Dec. 4 after he was caught in between a conveyor system and crushed while performing sorting operations.
OSHA cited Pittsburgh-based contractor Genco for failing to certify that a hazard assessment of the facility had been conducted before employees were assigned.
A message left for Genco was not immediately returned.
